- Fenestrating only the apical membrane, usually at the level of the foramen of Monroe, between the ventricle and the cyst
- ventriculocystocisternostomy (VCC)
- Fenestrating only the apical membrane to basilar fenestration toward the prepontine cistern (cyst-cisternostomy)
- A study conducted using MR-imaged CSF flow dynamics shows that fenestration of suprasellar cysts should be done in both the ventricles and in the basal cisterns, to prevent relapse of symptoms.
- Gui et al. concluded that endoscopic ventriculocystocisternostomy is far more effective than ventriculocystostomy

- Avoided due to a significantly higher morbidity, and the fact that the success rate does not go above 70%
