Treatment

  • Endoscopic fenestration
    • Standard modality of treatment
    • Techniques.
      • Ventriculocystostomy
        • Fenestrating only the apical membrane, usually at the level of the foramen of Monroe, between the ventricle and the cyst
      • ventriculocystocisternostomy (VCC)
        • Fenestrating only the apical membrane to basilar fenestration toward the prepontine cistern (cyst-cisternostomy)
        • A study conducted using MR-imaged CSF flow dynamics shows that fenestration of suprasellar cysts should be done in both the ventricles and in the basal cisterns, to prevent relapse of symptoms.
        • Gui et al. concluded that endoscopic ventriculocystocisternostomy is far more effective than ventriculocystostomy
  • Open procedures
    • Avoided due to a significantly higher morbidity, and the fact that the success rate does not go above 70%
