  • E: After a right pterional craniotomy and sylvian fissure dissection, the opercula are separated, exposing the insula. Circular sulci of the insula are identified all around, as well as Heschl’s gyrus posteriorly. Heschl’s gyrus is a good landmark as it points toward the posterior limb of the internal capsule and the atrium of the lateral ventricle if followed deep into the cerebral hemisphere. The M2 branches of the MCA are dissected off in order to find the best surface for an insular corticectomy and trajectory to the thalamic pathology.
