Vein of Galen disease
  • Hydrodynamic disorder:
    • graph LR A(AV shunting) --> B(venous HTN) --> C(reduced CSF absorption) --> D(hydrocephalus) --> E(inc. ICP) --> F(developmental delay) --> G(mental retardation) --> H(ventriculomegally) --> I(macrocranium) B --> A1(High outflow of blood) --> B1(Keep embryonic sinuses open) --> C1(Less blood flow thru jugular bulb and sigmoid sinus) --> D1(Stenosis of jugular bulb and sigmoid sinus) A1--> B2(If the blood finds alternate route to exit brain) --> C2(drainage through ophthalmic vein) --> D2(exophthalmos, epistaxis) B --> C3(Dec cerebral blood flow to normal brain) --> D3("Brain parenchyma subacutely and progressively destroyed (mainly in the white matter)") --> E3(Melting brain syndrome)
    • Try not to shunt these patients as -> shunting lower ICP -> increase cerebral blood flow -> less BP required to pump blood to brain -> global cerebral steal -> ischaemia
